Telehealth for Patients with Long Term Conditions

Problem

The project addresses the central issue of rising patient population with long term conditions and how the health systems will deal them with limited resources. Specifically NHS Great Yarmouth and Waveney is looking to utilise the learning from various telehealth pilots that have taken place in the UK and elsewhere and apply that on a large scale on an ongoing basis.

Solution

The Trust aims to enrol 2000 of their estimated 32000 patients with specific long term conditions in a telehealth programme over a number of years. The patients will be identified through hospital utilization data (i.e. frequent emergency and non-elective admissions and readmissions), Patients at Risk of Re-hospitalisation (PARR+) with input from their GPs,  community and hospital teams for suitability for home monitoring and offered a personalised telehealth system for self care. The purpose is to help these patients manage their condition, improve compliance with medications and intervene with appropriate support such as community matron visit or telephone support to prevent the symptoms becoming severe. The project will target patients with heart failure, coronary heart disease, chronic pulmonary obstructive disease and diabetes.
